In 1997, Dr. Cook received a Bachelor of Science degree in Public Health with an Industrial Hygiene emphasis and dual minors in Chemistry and Mandarin Chinese from Utah State University. He graduated from the Creighton University School of Dentistry in 2001 with a Doctor of Dental Surgery degree (DDS) and received his orthodontic training at St. Louis University Center for Advanced Dental Education where he received a Master of Science degree in Orthodontics.

During his free time, Dr. Cook enjoys spending time with his wife and their three children. He also enjoys exercise, studying Mandarin Chinese, and reading.
